How do I become a Cannabis patient?
A doctor must write you a marijuana recommendation. Some patients are reluctant to ask their primary physician for a referral. We can refer you to a physician to perform an independent, confidential evaluation.
Send the doctor’s recommendation to the Colorado Department of Public Health and Environment, along with your notorized application, CO license (or out of state ID with proof of residency) and $90 fee (money order or check made out to CDPHE). Notaries available at GroundSwell Boutique!
If you would like to designate GroundSwell Boutique as your caregiver our information will be filled out in boxes 16a-f of the Medical Marijuana Registry Application Form. we will help you with all the necessary forms and make copies for your records.
Once approved, you will receive a state-issued medical marijuana card and be included in the medical marijuana registry. Your ID card is valid for one year, after which you’ll need to apply again.
